ES Registration (Grades 1-3)

To complete the registration process you will need to do the following:

  1. Fill out the Student Registration Packet completely.
  2. Provide a copy of your child’s birth certificate.
  3. Provide current immunization records for your child. If your child has an appointment scheduled for the summer, please have the pediatrician’s office fax the report to (413) 743-8406. Immunization reports must be received prior to the first day of school.
  4. Provide proof of residency in the form of a utility bill, rental agreement, etc.
  5. Bring Student Registration Packet to the school office and complete remaining required forms (available in the office). Please call the school at 413-743-0876 or email jamiesmith@hoosacvalley.org to arrange a time.

Grade-Level Supply Lists
